A written request from your attending, referring, or requesting physician is required. This ensures that: The correct procedures and special instructions are followed. Important patient details are provided to assist the medical team. The requesting physician does not need to be affiliated with CHH. Appointment Guidelines for Diagnostic Procedures Booking in Advance: Appointments should be scheduled at least 2 to 3 days in advance of the preferred date. Ways to Book an Appointment: Unit Visit (What to Expect?) Staff will assist with the booking process, conduct a short interview, and provide a special instruction form outlining pre-procedure steps. Once scheduled, a confirmed appointment will be provided, and patients must arrive at least 30 minutes before the procedure. Phone Call Important: Inquiries made outside office hours or on closed days will not be entertained.

Authorized Claimants: Results can only be claimed by the patient or their legal guardian during the unit’s operating hours. Claiming by a Representative: If the patient wishes to authorize another person to claim the results, the following documents must be presented: Letter of Authorization signed by the patient, including the representative’s name. Copy of the patient’s ID and representative’s ID. Official receipt (For insurance payments, the account ledger must be presented). Pre-Procedural Instructions EEG and BAER Sleep Requirements: Limit sleep to 3 hours (from 12:00 AM to 3:00 AM) the night before the procedure. Once awake, avoid taking naps until the scheduled procedure. Personal Hygiene & Attire: Hair Care: Scrub the scalp using shampoo thoroughly. Do not apply hair products (e.g., conditioner, gel, oil, mousse). Clothing: Wear loose, comfortable clothing. Tight clothing is discouraged. Jewelry: Refrain from wearing necklaces, earrings, or any accessories. Dietary Restrictions: Fasting is not required. Avoid caffeinated and sugary beverages (e.g., soda, coffee, iced tea, sweet juices) on the day of the procedure. Other Reminders: Bring wet sanitary wipes. Arrive 30 minutes to 1 hour before the appointment. Late arrivals will result in referral cancellation. Present the physician’s written request and the prescribed mild sedative at the reception area. VER Personal Hygiene & Attire: Hair Care: Shampoo and scrub the scalp thoroughly. Do not apply hair products. Clothing: Wear loose, comfortable clothing. Tight clothing is discouraged. EMG-NCV-RNS-SSEP Personal Hygiene & Attire: Bathing: Take a full bath on the day of the procedure. Skin Care: Avoid applying moisturizers, oils, liniments, or lotions. Clothing: Wear loose, comfortable clothing for easy access to limbs. Tight clothing is discouraged. Dietary Restrictions: Fasting is not required. Other Reminders: Arrive 15 to 30 minutes before the appointment. Late arrivals will result in referral cancellation. Present the physician’s written request at the reception area.

Yes, please take note of the following: Companion Requirement: Patients who are receiving mild sedation, have special needs, or experience medical conditions such as dizziness, vertigo, vision problems, or balance issues should be accompanied by one or two able-bodied companions for assistance during the appointment. Submission of Physician’s Request: If a written request is unavailable (e.g., for online consultations), save a screenshot of the physician’s request and present it at the unit’s reception area. Post-Procedure Follow-Up: Once results are claimed, schedule a follow-up appointment with the attending neurologist or physician to discuss the findings. Important Notice: Patients must personally request a follow-up appointment. Unit staff are not authorized to schedule follow-ups or discuss procedure results.
