7.What are the Preparations before the procedure?
Cardiac Rehabilitation is a medically supervised program designed to improve your cardiovascular health if you have experienced heart attack, heart failure, angioplasty or heart surgery that includes exercises and health education.
It depends on what phase of the cardiac rehabilitation program you are enrolling in or have been advised to join. Please see the list for price difference:

A Cardiac Rehabilitation Specialist will determine which phase you will enroll in, considering your current condition, including factors such as your diagnosis, any type of surgery performed, and the time elapsed since the surgical procedure.
Phase 1 program: There are 8 sessions and a treadmill stress test after the 8th session.
Phase 2 program: There are 24 sessions and a treadmill stress test after the 24th session.
Phase 3 program: There are 48 sessions and a treadmill stress test after the 48th session.
Other Services offered:
Heart Failure (HF) Package - there are 24 sessions and a 6-minute Walk Test after the 24th session.
Chronic Kidney Disease (CKD) Package - there are 24 sessions and a 6-minute Walk Test after the 24th session.
Cancer (Onco) Package - there are 24 sessions and a 6-minute Walk Test after the 24th session.
Peripheral Arterial Disease (PAD) Package – there are 36 sessions and a Treadmill Stress Test after the 24th session.
Our Cardiac Rehabilitation Staff will send an SMS/text message on the provided contact number(s) a day before the scheduled appointment, including a specific time which must be strictly followed. Kindly confirm by responding to the text message.
If you wish to cancel or reschedule, please inform the Cardiac Rehabilitation Unit so the cancelled time slot can be given to other patients.
Should you wish to choose your own schedule, please communicate with our Cardiac Rehabilitation Staff to confirm availability.
The Treadmill Stress Test
Results will be available in 3 to 5 working days or depending on the availability of the Cardiac Rehabilitation Specialist.
Cardiac Rehabilitation Progress Report
Report will be readily available depending on the release of the Treadmill Stress Test Result.
NOTE: The Cardiac Rehabilitation Specialist will prepare the Cardiac Rehabilitation Progress report and the Treadmill Stress Test results. Once completed, the Cardiac Rehabilitation staff will collect the reports from the Cardiology Unit. The reports will be released at the Cardiac Rehabilitation Unit.
The 6-Minute Walk Test
Results will be readily available once the Cardiac Rehabilitation Specialist has done interpreting the data after the test.
For the Treadmill Stress Test, 6-Minute Walk Test, and Cardiac Rehabilitation Report, our Cardiac Rehabilitation staff will send an SMS/text message when your results are ready for pickup at the Cardiac Rehabilitation Unit.
The cardiac rehabilitation program typically takes 1 to 3 months to complete on average, assuming regular participation. Each session is approximately 1 hour long, depending on how well the patient responds to the exercises.
We recommend attending sessions three times a week with an every-other-day interval, in line with our operating days and hours.
Must arrive at the unit at least 15 minutes before the scheduled time.
Wear comfortable attire/gym clothes and a rubber shoe for safety reasons.
Have a light meal and bring water for hydration
Take maintenance medication as ordered.
Step 1: Proceed to the Cardiac Rehabilitation Unit located on the 2nd floor, Building C, just above the reception area near the General Radiology Unit or you may contact us at 255-8000 local 76215 for further inquiries.
Step 2: Present a referral note from a Cardiac Rehabilitation Specialist to the Cardiac Rehabilitation Staff.
NOTE:
If no Cardiac Rehabilitation Specialist is on board, the patient needs to secure a referral note from the Attending Cardiologist.
Patients must secure a schedule for an appointment with the Cardiac Rehabilitation Specialist and secure a referral note indicating which program to be enrolled.
Step 3: The Cardiac Rehabilitation Fellow will assess the patient, provide orientation, and obtain their consent.
Step 4: Register at our institution through our reception to have a Hospital Identification Number.

For patients with existing Chong Hua Hospital Record, please proceed to Step 5 after accomplishing Step 3)
Step 5: Cardiac Rehabilitation Staff will issue a charge slip to be paid at the Billing Section.
Step 6: Present the charge slip to the Billing Section for payment. Please contact our Billing Section for online transactions and queries.
Step 7: Present the receipt to the Cardiac Rehabilitation Staff at the Cardiac Rehabilitation Unit.
Step 8: Cardiac Rehabilitation Staff will secure the requirements needed.

8.1. Requirements
Referral Note from a Cardiac Rehabilitation Specialist.
Medical Certificate and Medical Abstract (From the Hospital/Institution where the patient previously admitted or surgical procedure was performed if there’s any.)
Discharge Instruction or Discharge Summary (From the Hospital/Institution where the patient previously admitted or surgical procedure was performed if there’s any or the Updated List of Discharge/Maintenance Medication and Diet.
Imaging and Blood Test Results including Lipid Profile, ECG Report, 2D Echo Report, Stress Test result and cardiac procedure report such as Coronary Angiogram, Percutaneous Coronary Intervention (PCI), or an Open-Heart Surgery) - Please provide us with the latest result.
Step 9: Cardiac Rehabilitation Staff will orient on the policy, protocols, safety reminders, preparations and scheduling of cardiac rehabilitation sessions.

Scheduling of Operating Room (OR) cases must be coordinated directly with the Operating Room team by the Surgeon, Resident, or Clinic Secretary.

Cardiopulmonary Clearance – Required to assess your heart and lung condition before surgery.
RAT (Rapid Antigen Test) Results –

First Come, First Serve (Per Shift Basis):

Late or Rescheduling Notice:

Chong Hua Hospital offers obstetric procedures for both inpatients and outpatients,
Walk- in for both OPD and In-patients as long as with physician prescription
Elective Cesarean Section are scheduled by attending Physicians, OB Residents and Clinic Secretaries.
Cardiotocogram Monitoring of the patient can last for about 30 minutes to 1 hour but varies depending on the status of the mother and baby.
Elective Cesarean Section and Normal Spontaneous Delivery depends on the case of the patient.

The estimated price for a normal delivery for private patients, excluding professional fees and any complications, ranges from P40,000.00 to P60,000.00. This estimate does not include fees for the baby.
The estimated cost for a Cesarean Section (CS) for a private patient without complications, excluding professional fees and the baby's charges, is approximately P80,000.00 to P120,000.00.
The estimated package price for a Normal Delivery for Obstetrics (OB) patients is P20,000. For availment, please inquire with the Social Services.
The estimated package cost for a Cesarean Section (CS) for Obstetrics (OB) service patients is P50,000.00. For availment, please inquire with the Social Services.
CTG fee is about P858.00 but subject to change without prior notice.
Phototherapy is available in CHH Neonatal Intensive Care Unit. If the baby has been discharged or is born from another hospital and would like to avail this service, it is available for P770.00/day. Patients have to be admitted to avail the service. On top of the P770.00/day use of phototherapy, additional fees will apply for the use of the NICU or regular room, as well as blood test to check the bilirubin levels.

Chong Hua Hospital allows the father of the baby or the mother/female guardian of a pregnant minor to witness the child’s birth and provide support as the mother goes through imminent delivery. Witnessing a baby’s birth is allowed only if the mother is assessed by her attending obstetrician to be low risk, anticipated to deliver normally, (not through caesarian section) and without any complications.
The Request to Attend the Delivery Form should be completed and signed by the attending physician and the father of the baby at least two weeks prior to expected delivery or during admission. The form should be submitted to the Delivery Room staff upon admission.

All procedures need to be scheduled ahead of time by the performing doctor. The patient needs to be seen by a Cardiologist (for Cath lab) or a Cardiovascular Surgeon (for CV-OR) for they will be the one who will schedule the patient with us.
We cater to both in-patients and outpatients. The Cardiologist or Surgeon will assess the patient if they are fit for an outpatient procedure or not so it’s important to be seen by the doctor.
NPO post-midnight for cases scheduled in the morning. For cases scheduled in the afternoon, patients may have light breakfast then go on fasting (4-6 hours) thereafter.
For OPD patients, please be at the unit 1 hour prior to the scheduled procedure.

Process Flow for RAI Therapy
Consultation with the NucMed Physician.
You will be given the preparations along with the quotation.
You will be scheduled for your therapy and must pay for the capsule one week prior to the scheduled date.
(Inpatient) On the day of your therapy, you will be asked to obtain the Admitting Orders from the Nuclear Medicine Department and present them to the Admitting Section.
Fasting for at least 2 hours is required prior to your therapy. The iodine capsule will be given to you in the patient isolation room (6B – Chong Hua Hospital Mandaue; 5A – Chong Hua Hospital Cebu)
You will be admitted for at least 3-5 days.
Post therapy Scan will be performed prior to discharge.

If the Emergency Room (ER) is full, patients who are already in the ER Triage Area will be prioritized based on the seriousness of their condition, not on a first-come, first-servebasis. When a bed becomes available, all patients waiting in the Triage Area will be reassessed based on how urgent their condition is and their vital signs using the ESI triaging system.

The OHCT program is a temporary care option for patients who are suspected of or confirmed to have COVID-19 and are waiting to be admitted to the Emergency Room (ER). This program applies to patients with mild and moderate COVID-19 symptoms, as assessed by the ER Resident.
An Internal Medicine (IM) Resident is assigned to oversee the OHCT program in the ER. The IM Resident will identify which patients are eligible to participate in the OHCT program.
The endorsing doctor and nurse will contact the ER station for the initial patient endorsement and to check for bed availability. If you are currently admitted in a healthcare facility within Region 7, the endorsing hospital shall submit a referral through the Department of Health (DOH) online system.
Your significant other or the person assisting you is advised to contact the Admitting Section for an appraisal of the financial aspects of your admission.

If you are availing of the Obstetrics (OBS) package for a normal delivery (Normal Spontaneous Vaginal Delivery or NSVD), you will be asked to pay an initial deposit of P5,000.00 at the cashier/ billing section once you’ve signed the consent form for BTL (Bilateral Tubal Ligation).
The remaining balance for the procedure will be settled upon your discharge.
As part of the package requirements, you will also need to secure and provide one unit of blood (any blood type), which may be donated before or during your hospital admission.
If you are undergoing a cesarean section, you won’t be required to make a deposit before the procedure. Instead, you will settle the full amount before discharge.
Please note that providing one unit of blood is still required, and any additional charges may apply at the time of discharge.
A payment of at least P1,500.00 is required at the cashier for each of your prenatal visits.

Repeated tardiness or missed appointments may also lead to forfeiture of your OBS package benefits.
In case of emergencies and the OPD is closed, you may proceed to the Emergency Room. Please don’t forget to bring your OBS card.

If you already have a record, the nurse will proceed with your registration.

Once the schedule is confirmed, the Attending physician will inform you of the schedule date and time.
On the day of your procedure, it is best to arrive 30 minutes before the scheduled time and approach the triage area.
The nurse will take your vital signs.
After triage, proceed to the nurse’s station where the nurse will register you.
After registration, you will be guided to the minor OR for your procedure.
If the procedure is covered under PhilHealth, you will be able to avail of PhilHealth benefits. The necessary papers for PHIC will be processed.

As the medication is a cold-chain medication, you can purchase the medication during the scheduled vaccine due dates.

Student VISA Medical Certification
If you are a foreign student seeking medical certification for your student visa, and you don’t have the necessary laboratory and diagnostic results, the OPD staff will guide you to comply with the required lab tests and diagnostics as mandated by your embassy.
Once you have the official results, return to the OPD for a medical checkup conducted by the Attending Physician.
If you are deemed medically fit, the Attending Physician will issue a medical certificate and complete the form required by your embassy.
The OPD staff will take the documents to Medical Records to have them sealed with the hospital’s logo for authenticity.
You will need to pay for the consultation charges. After payment, the OPD staff will release the sealed documents to you.
NOTE: We will only accept the results from Chong Hua Hospital. Patients who provide results from other facilities will be required to undergo or repeat the labs and diagnostics in Chong Hua Hospital.
It is best to call the OPD so that the staff can provide instructions on how to comply with lab and diagnostic exams. The OPD can also arrange the availability of the Attending Physician, as they are on an on-call basis.

Room and bed assignments are based on the date of admission. The admitting staff records these requests and allocates rooms, accordingly, considering both patient preferences and medical requirements.
Chong Hua Hospital requires an initial partial payment for all admissions. In the case of a scheduled, non-urgent admission, the payment must be made upon arrival. For urgent or emergency cases, the partial payment must be settled once the patient's condition has stabilized. Our admitting staff will provide you with the necessary information.
If you have a valid Letter of Authorization (LOA) or Letter of Guarantee (LOG) from your insurance provider, HMO, or affiliated company, you will not be required to make a partial payment. If an LOA/LOG is not available at the time of admission, a partial payment is required, which may be refunded upon discharge.
During your hospital stay, you will be asked to make progressive payments corresponding to the increasing bill amount. An updated statement of your account can be viewed via the online patient portal.
If you require an inter-hospital transfer, Chong Hua Hospital will assist in facilitating the transfer.
If you decide not to proceed with inpatient admission, Chong Hua Hospital will respect your decision and assist in transferring you to your preferred hospital.
Inter-Hospital Transfer
The patient or their significant other will be advised to coordinate with both the receiving hospital and Chong Hua Hospital.
The patient or their significant other will be advised to let the endorsing doctor and nurse call the ER Station for the initial endorsement of the patient and to check for bed availability. If the patient is admitted in one of the healthcare facilities in Region VII, the endorsing hospital will be asked to encode and make the DOH online referral.
The endorsing hospital shall coordinate with the receiving hospital through doctor-to-doctor endorsement.
The patient or their significant other will also be asked to call the admitting section to be appraised on the financial side of the admission.
Room category will depend on the present condition of the patient.

Non-COVID patients are permitted a maximum of two healthy adult visitors or watchers during admission.
For a confirmed COVID patient, one adult watcher is permitted during admission.
For non-COVID patients, except those in the Critical Care Units, you may have healthy visitors from 6 AM to 9 PM.
All visitors must observe physical distancing.
All visitors must wear a surgical mask at designated areas within the hospital (critical care units, emergency rooms, isolation rooms).
All visitors must practice cough etiquette. Ensure that a surgical mask is on when you cough or sneeze. Change the mask if it is soiled or damp. Throw the used mask in the yellow trash bin and wash your hands afterward.
Visitors must follow proper hand hygiene protocols such as frequent hand washing and the use of hand sanitizers.
Visitors experiencing symptoms of COVID-19, such as fever, cough, and difficulty breathing, shall not be allowed to visit patients.
Watcher/Visitor
Chong Hua Hospital requires patient watchers to be able-bodied adults. For the safety of both patients and visitors, minors and seniors are discouraged from serving as watchers. This policy is in place due to the increased risk of illness and falls for children and seniors within the hospital environment. However, minors (7 years old and above) and seniors are welcome to visit during designated visiting hours, provided they adhere to infection control guidelines.

Pap smear
Can be done by a Doctor (OBGYNE) in their clinic, or at the OPD Unit.
If patient opts to proceed to OPD Unit, follow these steps:
(1) Proceed to the laboratory for charging. A charge slip will be given to you. Once you have the charge slip, proceed to billing.
(2) At the Billing dept., present your charge slip and pay for the procedure. Once you have the official receipt, proceed to the Histopathology unit.
(3) At the Histopathology unit, present your Official Receipt in order to claim your pap smear kit. Once you have the kit, proceed to OPD.
(4) At the OPD unit, inform the unit in charge of your procedure. If there is no available slot for the day, a staff member will inform you of the next available schedule.
(5) After the procedure, bring your kit (specimen) back to the Histopath Unit. The result is available 7 working days after the specimen is submitted. Note: Result is not available to Patient Portal, the result is for pick up.

A: Have yourself evaluated first by our ophthalmologist of your choice for comprehensive examination and evaluation. Your doctor will explain to you your condition and the procedure.
A: Cataract surgery will be scheduled once you are properly evaluated by our ophthalmologist. All of our ophthalmologists can perform this surgery. A biometry is requested, and medical clearance might be needed depending on the assessment of your doctor before operation.

A: This will depend on the type of lens that is needed and will be used. Your doctor will help you decide which lens will suit your ideal vision, lifestyle and budget.

A sleep study or a polysomnogram (PSG) is a comprehensive test used to diagnose sleep disorders. A PSG is performed overnight in the sleep laboratory. It provides valuable data for diagnosing conditions like sleep apnea, narcolepsy, restless leg syndrome, and other sleep-related issues.
For Chong Hua Hospital Cebu: The Sleep Laboratory is located on the 11th floor of Building AB. You will stay in a dimly lit room equipped with a comfortable bed and furnishings. There will be no other patients in the room with you.
For Chong Hua Hospital Mandaue: The Sleep Laboratory is located on the 12th floor, Room 1223, of the Hospital Building.
Diagnostic Overnight Polysomnography (PSG):
A Diagnostic Overnight Polysomnography (PSG) is a sleep study conducted overnight to monitor and record various physiological parameters while you sleep. During the test, sensors and electrodes are placed on your scalp, face, chest, limbs, and sometimes other parts of the body. These sensors measure:
Brain activity (through EEG, or electroencephalography)
Eye movements (used to identify sleep stages)
Heart rate (through electrocardiogram or ECG)
Breathing patterns (measured by airflow sensors and a chest belt)
Blood oxygen levels (monitored through a pulse oximeter)
Leg movements (detected by sensors placed on your legs)
The goal of a Diagnostic Overnight PSG is to provide detailed information about your sleep patterns and to help identify any sleep disorders that may be affecting your health and well-being. The results help doctors create an appropriate treatment plan tailored to your specific condition.
Split-night PSG with CPAP Titration:
A Split-night PSG with CPAP Titration is a type of sleep study that is typically performed when moderate to severe sleep apnea is suspected. It combines the diagnostic process of a standard polysomnography (PSG) with a CPAP titration to determine the appropriate air pressure needed to treat the condition.
This is the protocol:
First Half of the Night (Diagnosis): During the first half of the night, a standard PSG is conducted to diagnose the severity of sleep apnea. This involves monitoring brain activity, oxygen levels, heart rate, breathing patterns, and other physiological functions during sleep. The goal is to identify the presence and severity of sleep apnea events, such as pauses in breathing.
Second Half of the Night (CPAP Titration): If sleep apnea is diagnosed and moderate to severe levels are confirmed, the second half of the night focuses on determining the optimal pressure for Continuous Positive Airway Pressure (CPAP) therapy. CPAP is a common treatment for obstructive sleep apnea. The sleep technician adjusts the CPAP machine to find the correct air pressure that prevents the collapse of the airway and keeps it open during sleep. The air pressure is administered through a mask that fits over the nose and/or mouth, allowing you to breathe freely and sleep without interruptions.
By the end of the study, the healthcare provider will have both diagnostic information and the required CPAP pressure settings to manage your sleep apnea effectively. This combined approach helps streamline the diagnosis and treatment of sleep apnea in one overnight study.
Therapeutic Study:
A CPAP Titration Study (or Therapeutic Study) is a sleep test that helps determine the right air pressure to treat sleep apnea using a CPAP machine. The CPAP machine helps you breathe easily while you sleep by keeping your airway open.
If you have sleep apnea, your airway can get blocked during sleep, causing you to stop breathing. This study helps figure out the best pressure level on the CPAP machine to prevent those breathing pauses.
You spend the night in a sleep lab, where the CPAP machine is used. The sleep technician adjusts the air pressure from the CPAP machine to find the right level that keeps your airway open.
While you sleep, they monitor how well you breathe and if your oxygen levels are steady. By the end of the night, the technician will know the best air pressure for your CPAP machine. This way, when you use the CPAP at home, it will be set to the right pressure to keep you breathing well and sleeping soundly.
This study helps find the perfect setting for your CPAP machine, so you can sleep better and treat your sleep apnea. The Biggest Reason for a Sleep Study – Excessive Daytime Sleepiness:
To schedule a sleep study, you must first see your physician, who will need to order the study. We cannot proceed with any sleep studies without a doctor’s prescription. Once you have the order, you can contact us to schedule an appointment at a time that works best for you.
If you do not have a physician yet, you may be referred to the Pulmonary Unit. The unit will coordinate with our sleep physicians. The Pulmonary Unit is located on the 3rd floor of Building C. You can contact them at 0968-875-1324 or through the CHH Fuente Trunk Line at (032) 255-8000, local 76240.
For cancellation and rescheduling, please make every effort to keep your scheduled appointment. A sleep technician and a room have been reserved specifically for you. If you need to cancel or reschedule, kindly notify us at least three days in advance to avoid any inconvenience.
It usually takes about one to two weeks for the results to be processed. After the study, a sleep technician will score every minute of the data, and a sleep doctor will review it for final interpretation. The doctor will assess whether a sleep disorder is present, determine the type of disorder, and create a treatment plan if needed.
You can claim your results at the Pulmonary Unit. For patients from outside Cebu or the province, the results will be sent to your email by the Unit Manager.
The sleep study typically begins around 8:00 PM and continues until approximately 6:00 AM. While there is no set time for you to fall asleep, we encourage you to go to sleep as soon as possible to maximize the amount of sleep time available for observation.
Before the Procedure:
Take a shower before going to the sleep lab
Avoid using gels, creams, oils, or any other hair products.
Do not apply facial creams, body lotion or body cream.
Wear or bring comfortable sleepwear.
Avoid napping during the day.
Refrain from engaging in strenuous physical activities for at least 6 hours before the procedure.
Avoid drinking caffeinated or alcoholic beverages.
Shave any facial hair at the chin area.
Do not take sleeping pills.
Have dinner before heading to the sleep lab.
Bring your doctor's prescription.
Bring the approved charge slip.
Only pediatric patients are allowed to have family members stay with them in the room of the sleep laboratory.
If you have insurance, please secure a valid LOA (Letter of Authorization) before the procedure date.
The purpose of a sleep study is to collect data during your sleep, such as brain activity, heart rate, and breathing patterns. The following equipment will be used:
Electrodes will be attached to your scalp (forehead, behind the ears, around the eyes, and below the chin) to monitor brain activity, eye movement, and muscle tone. These electrodes are secured with adhesive paste and cotton.
Electrodes on your legs, below the knees, will monitor leg and body movements.
Chest belts will be placed around your chest and abdomen to monitor nasal breathing.
Nasal tubing, similar to oxygen tubing, will be used to monitor nasal breathing.
A soft finger wrap will measure your oxygen levels throughout the night.
For most patients, the sleep study is at least partially covered by their insurance policies. However, it’s important to verify the details with your insurance company or policy provider. Many insurance companies require pre-approval for sleep studies.

The TB DOTS Unit provides the following services:
Enrollment for TB treatment (free anti TB medications c/o DOH)
Continuation of treatment (for patients enrolled elsewhere who want to transfer to CHH DOTS Clinic)
TB preventive therapy
Diagnostics – sputum AFB smear, sputum GeneXpert processing, PPD (with corresponding fees)
To enroll in the TB DOTS Program, please prepare and present the following:
A National Tuberculosis Program (NTP) Referral form or a referral written on a prescription pad from your attending, referring, or requesting physician
Your PHILHEALTH number
If you are not yet a PHILHEALTH member, our nurse can assist you by referring you to the PHILHEALTH personnel inside Chong Hua Hospital.
Official test results, such as chest X-ray, sputum GeneXpert, and other laboratory results.
We accept both inpatient referrals (from admitted patients) and outpatient referrals (from the doctors at Medical Mall and non-Chong Hua Hospital accredited physicians).
We accept walk-in referrals from 10:00 AM to 6:00 PM, with a cut-off time of 4:00 PM.
For succeeding visits, you will be scheduled according to your treatment plan.
You may also schedule a consultation whenever needed if you experience side effects from the treatment (medication).
Unit Visit Procedure
TB DOTS Clinic Process Guide
Reason for Visit: When you arrive, the nurse will ask about the reason for your visit.
For Enrollment: If you are enrolling in the TB DOTS program, please provide your NTP Referral Form or a referral written on a prescription pad by your attending physician, along with your diagnostic results (chest X-ray, lab tests, GeneXpert, etc.).
Vital Signs & Registration: The nurse or nursing assistant will check your vital signs and weight and register you in the HIS system. Please present one valid government-issued ID.
Orientation & Consent: The nurse will explain the TB DOTS procedure, services, and fees and ask you to sign a consent form for enrollment.
Waiting Area: After the orientation, you will wait while the nurse informs the resident in charge.
Consultation & Treatment Plan: The resident doctor will conduct a consultation. Afterward, the nurse will discuss your care and treatment plan.
Billing & Payment: The nurse will provide a charge slip and guide you to the Billing Office for payment.
(Best option for payment is at 5th Floor Bldg. C OPD open until 5 PM. After 5 PM, proceed to the Main Billing Department at Ground Floor.)
Medication & Follow-Up: Return to the center to receive a two-week supply of TB medication and schedule your next follow-up visit in two weeks. You will also receive a TB DOTS brochure for reference.
Follow-Up Visits
The nurse will retrieve your chart, check your vital signs and weight, and have you wait for your consultation.
The resident doctor will assess your treatment progress and any side effects of the medication.
You will receive a charge slip for payment at the Billing Office.
Return to the center to receive another two-week supply of TB medication and schedule your next visit. This continues until your treatment is completed.
Initial Visit (No Referral / Walk-In Patients)
The nurse will check your prescription for diagnostic tests (e.g., Gene Xpert) and guide you on proper sputum collection.
You will be directed to the Laboratory Department for test payment and specimen collection.
Once results are available, return to your attending physician for consultation.
Mantoux (PPD) Test Process
Present your doctor’s order for the test. The nurse will validate it and direct you to the Billing Department for payment.
After payment, return to the center for the procedure:
Pediatric patients: The pediatric resident will perform the test.
Adult patients: The nurse will perform the test.
You must return 48 to 72 hours later for the test reading.
You need to buy the PPD reagent from CHH Pharmacy or an outside pharmacy.
Visit the unit from Monday to Wednesday to avoid test readings falling on a weekend.
Regular TB Medicine Pick-Up
Patients are required to visit the center every two weeks for TB medicine refills.
During each visit, the resident will assess for side effects or compliance issues with TB medication.
If any adverse reactions occur, you will be advised to consult your attending physician.

For new clients, a list of basic exercises will be given to them valid for 2-3 weeks. They are guided by coaches accordingly and depending on familiarity with exercises and/or machines. Scheduling for Treatment
After consultation, the patient will then be recommended for therapy. The patient will now schedule through the unit assistant for future therapy sessions, subject to schedule availability. This is listed/noted on their personal appointment booklet
Patients are encouraged to arrive at least 30 minutes before their scheduled therapy time to process billing etc.
In the event a patient is late, the patient has the option to continue therapy however at a shortened time or advised for re-scheduling. No extension to the next hour is allowed.

Results of your examinations and procedures will be prepared by our staff and are readily available after three working days except with biopsy procedures which will be completed in five working days. The EHU staff will inform you if the result is ready for pick or may opt through courier. The tests can also be viewed using the hospital’s portal app.
HMO coverage for executive checkup depends on the premium and benefits provided in your HMO account. You may contact your HMO and inquire about your benefits. Once approved by your HMO, they will contact EHU to schedule you on your available time.
Other HMO companies will provide you with a letter of guarantee addressed to Chong Hua Hospital either in Cebu or Mandaue and please bring this letter to your appointment schedule.
BLOOD CHEMISTRY TESTS – Fasting is mandatory at least 10 hours prior to blood extraction. You may drink sips of water but there should be NO CALORIC INTAKE.
STOOL OCCULT SAMPLE- If your plan requires you to submit a stool sample, please be reminded that red meat and foods containing iron should not be consumed for at least three days prior to your appointment schedule.
URINE SAMPLE- Collection of urine samples can be done within the hospital premises. There is no need to collect the sample at home unless a 24-hour urine collection is requested.
24 HOUR URINE COLLECTION (This test is collected if your doctor requires this test done. If not indicated, NO need to follow the steps written below:
STEPS FOR 24 HOUR URINE COLLECTION
Prepare 3 pieces of clean and empty 1-liter water bottles.
Place a label on each bottle containing the patient’s NAME, BIRTHDATE, DATE & TIME of the start of urine collection.
Disregard the first urine sample, then start timing for the next 24 hours.
All urine voided thereafter must be collected completely and placed inside the prepared clean water bottle. Take care not to spill the urine. A clean plastic funnel may be used for this purpose.
Consecutive urine samples collected may be added to the previous samples in the same bottle. Cover bottles tightly. Keep it in a bucket with ice to keep it cool.
A REQUIRED blood sample for plasma creatinine is extracted at the 12th hour of the urine collection.
Submit all urine collected for 24 hours.
Keep the bottles cool, ideally placed on ice during transport to the laboratory.
TRANSVAGINAL ULTRASOUND AND PAP SMEAR- If this test is included in your package or requested by your doctor, please avoid sexual contact for at least three days. If you have your period, you may opt to reschedule the said procedure.
IMAGING (ULTRASOUND, SCAN OR MRI) – If any of the following tests is requested, kindly take note of the following:
WHOLE ABDOMEN/UPPER ABDOMEN ULTRASOUND/CT SCAN OR MRI – 4-6 hours fasting is required
LOWER ABDOMEN ULTRASOUND OR SCAN – Fasting is not required but you may be instructed not to void for a few hours in some of the test procedures. Ask the staff regarding this test.
BREAST ULTRASOUND/MAMMOGRAM – No preparations needed but preferably 2 weeks after menstrual period.
TREADMILL STRESS TEST (CARDIO EXERCISE) – This is a special non-invasive cardio procedure which requires patients not take beta blocker medications at least two days before the procedure. Please inform the staff of your current medications prior to the appointment or procedure. In performing this procedure, it is best to wear comfortable clothes like the following:
Jogging or loose pants or shorts
T-shirt
Rubber or running shoes
Extra clothes or face towel
PULMONARY FUNCTION TESTS – This test is often included in an executive checkup to assess health and respiratory function. If this test is part of your plan, you need to have some preparation prior to conducting the procedure. This test will be rescheduled if you are developing respiratory symptoms.
Eat a light meal.
Avoid smoking for at least 12 hours before the test.
For patients using respiratory inhalers or nebulizing medications, please put on hold any inhalers or nebulization medication for

4-6 hours for nebulization medication

All patients on NGT Feeding are provided with Oral Nutrition Supplements, prepared by the dietary department Enteral formula is made from commercial products in the food and Nutrition Services according to the physician’s diet order. The enteral formula can also be prepared from commercial products bought and brought to the department by the patient or significant others facilitated by the nursing staff.
Preferably, patients will need a referral from their APs. If there is no referral, the patient will be assessed by the dietitian, considering medical prescription and laboratory results.

Yes, only if you provide supporting documents (medicine receipts, payment to doctors etc.) within the period indicated in your statement of account covered by the government assistance or if you get confirmation from the government agency issuing assistance that excess payment to the hospital will be given to the patient and will not be returned to the government agency.
Another option, that you can benefit from the excess payment is to offset the refundable amount to your future laboratory, out-patient, in-patient, ER or Pharma transactions.
Please note, government agencies will normally issue a Letter of Guarantee only to the extent of the patient’s outstanding balance to the hospital. It will not issue more than the patient’s payable to the hospital.

Society of NEUROLOGY and UROLOGY are not affiliated with any insurances, with this, the Issuance of LOA for Consultation is not possible.

Philhealth Benefits Eligibility Form (PBEF) signed by member
On the day of the procedure, the PBEF with 'YES' eligibility will be generated as required by PhilHealth. The patient must present their PhilHealth ID, Member Data Record (MDR), or PhilHealth number—whichever is available—along with a valid government-issued ID for identification purposes.
The Claim Signature Form (CSF) must be signed by the member. If the member is employed but the PBEF eligibility result is 'NO,' the CSF must be signed by the employer.

For NON-Members

Supporting documents establishing the relationship to the member/patient must be provided. The representative must also be of legal age.

Changing of Civil status
Fill out the PMRF, attach the marriage contract with the registry number, and submit it to the PhilHealth office for updating within the confinement period.
Adding additional dependent
For Spouse: Fill out the PMRF, attach the marriage contract with the registry number, and submit it to the PhilHealth main office within the confinement period.
For children: Fill out the PMRF, attach the birth certificate with the registry number, and submit it to the PhilHealth main office within the confinement period.
For a newborn child: Fill out the PMRF, attach a certified true copy of the Live Birth signed by the attending physician, medical records clerk, and parents, then submit it to the PhilHealth office within the confinement period.
Refund
The hospital has 60 days from the date of discharge to process and submit claims to PhilHealth. Similarly, PhilHealth has 60 days from the date of receipt to review and pay the claim. It is advisable to follow up on the refund after four months from the date of discharge.
PhilHealth also informs its members through their registered mobile number when the claim payment has been made.

For babies born to married parents, the Medical Records staff will assist the parents in registering the Certificate of Live Birth with the City Civil Registrar's office. The parents can collect the registered Birth Certificate 2 to 3 weeks after the baby's discharge.
For babies born to unmarried parents, the parents must personally register their baby's Certificate of Live Birth at the City Civil Registrar's office. Parents are required to submit a notarized affidavit as well as a notarized Certificate of Live Birth to the City Civil Registrar's office. Affidavit will be provided by the Medical Records staff.

Urine sample collection typically follows these steps:
Wash hands thoroughly before collecting the URINE sample
Clean the genital area using wipes or tissue (for females, wipe from front to back; for males clean the tip of the genital area).
Use a sterile container provided by the laboratory.
Collect a midstream urine sample: Start urinating into a toilet then stop and collect urine in a container (this helps avoid contamination from the initial flow of urine)
Fill the container with the recommended volume of urine usually 30-50ml.
Close the container tightly and label using the complete patient's name, date of birth and date and time of urine collection.
Deliver the urine sample to the laboratory within an hour from specimen collection.

X-RAY – if taken by morning the result will be read and released the same day in the afternoon.
